The authors report a series of 51 cases of reoperation following a Keller procedure for hallux valgus. Clinical problems were generally lateral metatarsalgia, stiffness or ankylosis of the 1 st metatarsophalangeal and/or deformity in extension, varus or valgus of the great toe. Operations used were transposition of the tendon of flexor hallucis longus to the first phalanx (6 cases), arthrodesis of the 1st metatarsophalangeal (23 cases), metatarsal alignment (14 cases) and osteotomies of the bases of the metatarsals (8 cases). Results were considered very good in 5 cases, good in 21 cases, acceptable in 22 cases and there were 3 failures.
