The aims of this intervention are correction of valgus deformity, saving the articular surface, obtaining a correct metatarsal alignment; it can be done alone or, more generally, in association with total or partial metatarsal alignment. The technique saves the insertion of the intrinsic muscles on the PHI and mantains to power of flexion of the hallux. Resection of the metatarsal head is first done; the head is modelled in shape of buffer, using the better part of the articular surface. A cylinder is then excavated into the diaphysis of the first metatarsal; into this is inserted to force the stem of the cephalic portion. The assembling is stable. Other operations are generally associated (resection of metatarsal head, arthroplasty according to Regnauld, implants) in lesser metatarsals. Indication was slinghtly or not at all reducible hallux valgus with suitable articular cartilage. Valgus deformity was generally joined with subluxation of luxation of contiguous M-Ph joints. Twenty-five patients (27 feet) aged 32 to 69 (average 56) underwent a pulling-back arthroplasty in the period July 1985-december 1987. Generally good results were obtained regarding correction of valgus deformity, restoration of power flexion of the hallux and relieving of metatarsalgia. The loss of articular motion of first M-Ph joint was not greater than 1/3.
